URGENT - Lost Card

Hello, 

I lost my card earlier today and I would like to replace it.
In order to do that, I saw that I need to had linked my card to an account in “ov-chipkaart.nl ”.

However, I cannot do that now since I do not know the expiration date of my card.
Can anyone please help me on this?
