I lost my OV-chipkaart, but it was not linked to my account. In order to block and replace it I need to have the expiry date, which I do not have. Is there a way I can still replace the card without the expiry date?

If you have an NS season ticket, then you can contact NS customer service by phone, chat, Facebook or Twitter to get the expiry date. In any case, the OV-chipcard customer service can also tell you this date.
