I lost my personal OV before I got the chance to register it and sadly I do not have any information of the card. Is there a way to cancel the subscription or simply block the old card and order a newer one

I'd recommend contacting the OV-chipcard service to have your card replaced in this case, then you'll be able to use it again in about a week's time. Contact our customer service for alternatives on how to use your product from us now if you have one and if you don't have the data of your card, you can also ask us for this
