I’m a OV-chipcard user and on my monthly transactions list is a wrong transaction. How can I get refund for following mistake and how did that happen??

Looking at the times I don't think there actually was a check in missing. It seems like TS managed to check out twice somehow. Between checking out in Zwolle at 9:39 and checking out there again he can't possibly have traveled anywhere and back.
